Most major insurance companies have dozens of smaller subsidiary providers that are not listed here, so if you don't see your insurance provider on the list, please contact your insurance provider. Patient Financial Responsibilities

· The patient (or patient’s guardian, if a minor) is ultimately responsible  for the payment for treatment and care. We strongly recommend that you contact your insurance provider to verify your coverage and deductible.  

· We will bill your insurance for you.  However, the patient is required to    provide the most correct and updated information regarding insurance.

· Patients are responsible for payment of copays, coinsurance, deductibles and all other procedures or treatment not covered by their insurance plan

· Coinsurance, deductibles, and non-covered items are due 30 days from receipt of billing.  We reserve the right to clear unpaid balances with on file credit card as appropriate.

· Patients may incur, and are responsible for payment of additional charges, if applicable.  These charges may include:

· Charge for returned checks $20.00

· Charge for missed appointments without 24 hours’ notice $50.00 to be charged to on file credit card at time of missed appointment
